Rhinoplasty, chin, facelift: planning one trip instead of three.

Profileplasty, rhinoplasty with blepharoplasty, rhinoplasty with deep plane facelift. What can safely share one anesthesia, what is better staged, and how long international patients actually stay.

Yes, rhinoplasty is often combined with other facial procedures in a single operation. The most frequent pairing is profileplasty, meaning rhinoplasty plus chin augmentation with an implant or a bone advancement, because nose and chin define the profile together. Blepharoplasty, otoplasty and facial fat grafting also combine well. Rhinoplasty with a deep plane facelift is possible in selected patients, but it lengthens anesthesia and recovery and is not right for everyone. Dr. Guilherme Sella defines what can share one trip after a full consultation, considering your anatomy, health status and total operating time.

Which procedures are commonly combined with rhinoplasty

Traveling for surgery makes people ask a practical question. If I am already going to be in Brazil, under anesthesia, with time off work, can I treat more than one area?

Often yes. Facial surgery combines well when the procedures share the same surgical field, the same positioning and a reasonable total operating time. The combinations most frequently discussed in consultation are:

  • Rhinoplasty plus chin surgery, known as profileplasty, with a silicone implant or a sliding genioplasty that advances the bone
  • Rhinoplasty plus upper or lower blepharoplasty, to address heavy upper lids or lower lid bags
  • Rhinoplasty plus otoplasty, common in younger patients who have both complaints
  • Rhinoplasty plus facial fat grafting or Nanofat, to restore volume in the midface and temples
  • Rhinoplasty plus functional nasal surgery, meaning septoplasty, turbinate reduction or endoscopic sinus surgery, which is not really a second surgery but the same operation treating breathing

Functional work is almost always part of the plan, not an add-on. Dr. Sella is an otolaryngologist, so septum, turbinates and nasal valves are assessed in the same consultation as the aesthetic plan. You can read more about the technique options in rhinoplasty and preservation rhinoplasty.

Profileplasty: why the chin changes how the nose looks

The eye reads the profile as a single line that runs from the forehead to the nose, lips and chin. A nose can look large simply because the chin is set back. Reducing the nose alone, in that situation, may still leave the profile unbalanced.

This is why chin assessment is routine during rhinoplasty planning. Standardized photographs and digital simulation show how the profile behaves when the chin projection changes. Two options are used:

  • Chin implant, placed through a small incision inside the mouth or under the chin, usually faster and performed under local anesthesia with sedation when done alone
  • Sliding genioplasty, a bone advancement that uses your own mandible, useful for larger corrections or when vertical height also needs adjustment

Adding a chin procedure to rhinoplasty usually adds modest operating time and very little to recovery. Swelling and numbness in the lower lip region can last a few weeks. The decision between implant and bone advancement is individual and defined in consultation.

Rhinoplasty and deep plane facelift: can they be done together

They can, in selected patients. A deep plane facelift with deep neck work and a structured rhinoplasty are both long operations, so the main limiting factor is total anesthesia time rather than the surgical field itself.

Points considered before combining them:

  1. Your general health, cardiovascular status and preoperative workup
  2. The complexity of each procedure, since a revision rhinoplasty with rib cartilage is a different scenario from a primary preservation case
  3. Postoperative swelling, which is cumulative and makes the first weeks more demanding
  4. Your availability to stay longer, because a facelift requires suture removal and closer follow-up than a nasal cast
  5. Whether you have an acceptable support person with you during the first week

When the combination would create an excessively long operation, staging is the safer route. Many patients do rhinoplasty first and return later for the face, or the opposite, depending on what bothers them most.

How combining changes operating time and recovery

The table below gives orientation ranges. Real numbers depend on your anatomy and are defined after examination.

CombinationSettingOperating timeSocial recoveryTypical stay in Maringá
Rhinoplasty aloneHospital, general anesthesia3 to 5 hoursCast for about 7 days, then tapingAbout 7 days
Rhinoplasty plus chinHospital, general anesthesiaSlightly longerSimilar to rhinoplasty, more lower face swellingAbout 7 days
Rhinoplasty plus blepharoplastyHospital, general anesthesiaLongerEyelid stitches removed in the first week, more bruising7 to 10 days
Rhinoplasty plus fat graftingHospital, general anesthesiaLongerSwelling settles over several weeksAbout 7 days
Rhinoplasty plus deep plane faceliftHospital, general anesthesiaConsiderably longerCumulative swelling and bruising, longer follow-upDefined individually, longer than 7 days
Revision rhinoplasty with ribHospital, general anesthesiaLonger than primaryChest site discomfort in the first days7 to 10 days

For rhinoplasty, bruising around the eyes can persist up to around 15 days, the final result settles between 6 and 12 months, and up to 18 months in thick skin or revision cases. Combining procedures does not change that timeline for the nose.

Planning one trip to Maringá for combined surgery

The sequence for patients who live abroad or in another Brazilian state is the same, with a longer stay when more than one procedure is planned:

  1. Teleconsultation with photographs and a discussion of goals, techniques and limits
  2. Preoperative tests done in your own city, reviewed by the team before travel
  3. Arrival one to two days before surgery, with an in person examination, standardized photographs, digital simulation and high frequency skin ultrasound when indicated
  4. Surgery at Saine Health Complex, a boutique hospital in Maringá with adult and neonatal ICU and intensivists on site around the clock, with 12 to 24 hours of admission
  5. Local recovery until the cast is removed and the surgeon clears you to fly
  6. Follow up at 1, 3, 6 and 12 months, in person or online

The team communicates in Portuguese, English and Spanish and issues documentation in English or Spanish on request. Maringá has direct flights from São Paulo and Curitiba. Practical details are in international patients.

What is usually better staged

Some things should not be squeezed into a single trip:

  • Complex revision rhinoplasty plus a long facial procedure, when total anesthesia time would become excessive
  • Surgery in a patient with poorly controlled systemic disease, until clinical optimization is done
  • Procedures with conflicting recovery rules, such as treatments requiring sun exposure restrictions combined with activities you cannot postpone
  • Fractional CO2 laser over areas recently operated, which is usually scheduled separately
  • Filler placed shortly before rhinoplasty, which can distort tissue planning and is assessed with nasal ultrasound
Combining procedures should shorten your travel, not stretch your surgery beyond what is reasonable.

Safety, anesthesia and who decides

Every combined plan is a balance between convenience and safety. Longer operations mean longer anesthesia, more swelling and a slower first week. That is why preoperative assessment, anesthesia evaluation and a hospital with intensive care structure matter more in combined cases than in isolated ones.

Dr. Sella holds a direct doctorate in Morphophysiology of Facial Structures from HCFMRP-USP, is certified by the European Board in Facial Plastic and Reconstructive Surgery and has published on dorsal preservation in indexed journals. The surgical plan, however, is built around your anatomy, not around a technique preference. Frequently asked questions

Is it safe to combine rhinoplasty with a facelift?

It can be safe in selected patients, but the main limit is total anesthesia time and your general health. When the combination would make the operation excessively long, staging the procedures is the safer choice, and that decision is made after a full preoperative assessment.

What is profileplasty?

Profileplasty means treating the nose and the chin in the same operation, since both define the facial profile. The chin can be augmented with an implant or with a sliding genioplasty that advances your own bone.

How long should I stay in Brazil for combined surgery?

For rhinoplasty alone, most patients stay around seven days until the external cast is removed. Combined procedures such as eyelid surgery or a facelift usually require a longer stay, defined individually by the team before you book flights.

Does combining procedures increase the cost proportionally?

Combining procedures shares hospital time, anesthesia and travel, which changes the overall structure of the estimate. The budget is only provided after the consultation, since it depends on technique, operating time and grafts.

Can functional nasal surgery be done at the same time as rhinoplasty?

Yes. Septoplasty, turbinate reduction and nasal valve correction are routinely performed in the same operation as the aesthetic rhinoplasty, since breathing and shape depend on the same structures.

Will the recovery of each procedure make the other worse?

Swelling and bruising are cumulative, so the first week after a combined operation tends to be more demanding. The nasal timeline itself does not change, with the definitive result settling between six and twelve months.

What is the difference between a chin implant and a sliding genioplasty?

A chin implant is a preformed silicone piece placed over the bone through a small incision inside the mouth or under the chin, and it mainly adds projection. A sliding genioplasty cuts and advances a segment of your own mandible, which allows larger corrections and also adjusts vertical height when needed. Both can be performed in the same operation as rhinoplasty, and the choice depends on how much movement your profile needs. Dr. Sella defines this after examining you and reviewing the standardized photographs and simulation.

Can I have eyelid surgery at the same time as my rhinoplasty?

Upper and lower blepharoplasty are frequently combined with rhinoplasty because they share the same surgical field and positioning. The main practical effect is that bruising around the eyes tends to be more visible in the first days, since both procedures cause periorbital swelling. Most patients still follow the usual rhinoplasty timeline, with the external cast removed around seven days. Whether your eyelids are a good indication for surgery is decided during the consultation in Maringá or by teleconsultation.

Is there a limit to how many procedures can be done in one anesthesia?

There is no fixed number, but total operating time, your health status and the preoperative evaluation set the limit. Rhinoplasty alone usually takes three to five hours, so adding long procedures such as a deep plane facelift changes the planning considerably. Surgeries are performed at a hospital with adult intensive care and anesthesiologists, which allows longer combined cases when they are appropriate. The final plan is defined in consultation after clinical and laboratory assessment.

Should I treat my chin with filler instead of having chin surgery with my rhinoplasty?

Hyaluronic acid in the chin is temporary and has limited capacity to create projection in a receded chin. When the profile needs a real structural change, an implant or a bone advancement gives a stable result in the same operation as the rhinoplasty. Filler can be an option in selected cases with small adjustments, and Dr. Sella uses ultrasound guidance when it is indicated. The comparison for your anatomy is made during the consultation.
